In my research of the history of asthma I have come across a very interesting phenomenon, or a conundrum perhaps. It is that asthma was never considered a fatal disease before the 1900s. Why is that?

The authors of "Fatal Asthma" ask it this way: "If the under treatment of asthma is invoked as an important cause of fatal asthma, then we must explain why people are dying from asthma now, when deaths from the ailment were negligible in the past."

You'd think with all the modern asthma wisdom and all the new medicines on the market today that there would be fewer asthma related deaths and not more.

Some have proposed that pollution may be the cause, but many scientists rule this out because prior to the 1900s air was polluted by coal and wood burning stoves, horses roamed the streets leaving their droppings, streets were of sand that caused dust, people did not shower daily and were often breeding grounds for diseases that triggered asthma. Plus people smoked frequently in public buildings and homes.

All of those obstacles to clean air have been removed. Yet, "Still there is genuine concern about newer sources of pollution, because the asthma rates have been rising in some American cities during the same period as pollution legislation has led to cleaner air. And this remains a possibility.

Another possibility, or theory, is that asthma did not become a fatal disease until 1903 when epinephrine (adrenaline) was first used in hospitals to treat asthma. One doctor observed that it's "possible that mortality from asthma first appeared when adrenaline became available... The pattern of events of no adrenaline, no deaths; some adrenaline, some deaths..."

The conundrum of doing such research, however, is that there is no research of untreated asthma. For the most part, if a doctor has a patient who is suffering with asthma the patient is going to be treated.

However, it wasn't until after "isoprenaline was introduced in a nebulizer formulation during the 1940s when mortality began to increase..."

In the 1960s "asthma mortality increased dramatically." Scientists evaluating the data determined the cause was "due to new methods of treatment. Interest focused initially on the possible role of pressurized metered-dosed-beta-agonsit aerosols, which had been introduced in the early 1960s."

In the 1970s there was a significant jump in New Zealand, and this was associated with a long acting beta agonist fenoterol that patients were allowed to use at home. When some patients were having trouble breathing, they'd often use it more than prescribed. This medicine, like epinepherine and isoprenaline, can have fatal cardiac effects if used in too high of doses.

The debate of whether asthma related deaths were caused by the routine use of these early beta agonists, or if it was that these meds were abused by asthmatics during periods of life threatening asthma "in which the cardiac side effects are likely to be particularly harmful in the presence of severe hypoxia."

However, unlike Albuterol, both of these classic asthma rescue medicines were not selective to the lungs only. They had an equal effect on the heart. When warnings were given out of the adverse side effects of overusing these medicines, the death rate dropped.

Another theory of why there are more fatal asthma episodes now is that asthma was under diagnosed, which may also be the reasoning for the trend the number of asthma cases nearly doubling between 1985 and 2009.

However many scientists have ruled this out. Still it's a theory worth considering.

I would guess the increased incidence of fatal asthma would be due to a combination of all three theories mentioned above, but I would lean toward the theory of modern medicine overuse as the one that makes the most sense to me.

I haven't seen trends after 1998, but I do know that many recent fatal asthma episodes have been linked to Symbicort and Advair, or the long acting bronchodilator within these meds. As was the case with fenoterol in New Zealand in the 1970s, scientists aren't sure if it is the drug itself that is causing fatal asthma, or that the drug is being used as a rescue inhaler for life threatening asthma.

The FDA has recently advised doctors that modern long acting bronchodilators such as those included in Symbicort and Advair (Serevent and Formoterol) are safe and that the benefits far outweigh the risks in managing asthma. Which makes one think the FDA believes abuse of these medicines is the reason for the recent spike in Fatal Asthma.

And, like in the 1970s, I believe, as the word gets out that Symbicort and Advair can have fatal consequences if not used exactly as prescribed, the asthma death rate will again go down once again.

No one knows for certain why episodes of fatal asthma have increased since 1900, and particularly since 1940, but most of the trends point toward the same meds doctors prescribe to treat acute and chronic asthma.

There is a lot of controversy and debate regarding the point whether tonsils should be removed or not in a person suffering from asthma. Different doctors have different opinions in this matter. Some advocate tonsillectomy (removal of tonsils) while others do not. It is therefore important to understand the role of tonsils in the human body so that adequate judgement can be made in this regard.
Tonsils make the first line of defence in the human body. They are present on either side of the pharynx. Pharynx is the middle portion of the throat. Why tonsils make the first line of defence is because they try to trap any germs and foreign bodies that may enter the body through the mouth and the nose. After trapping them the tonsils also produce protective antibodies with the help of their lymphatic cell structure that forms the main body of the tonsils. These antibodies circulate in the blood and fight the foreign organisms that somehow pass into the blood stream. So tonsils on one hand act as a mechanical guard and on the other hand they act on the molecular level also to protect the body. The antibody forming tissue is present in other parts of the body too but tonsils are first line of defence of the human system.
The body of a child is exposed to many different kinds of germs for the first time. The body is not used to these germs and needs to be protected against them. The tonsils work vigorously and in some children get abnormally enlarged in the process. If a child suffers from cough and wheezing his breathing is as it is strained. If this is due to some allergic reactions it may be accompanied with sore throat and fever as well. If the allergy persists or may subside and reappear again and again the tonsils too get infected while protecting the body.
In such a scenario the infected tonsils may need to be removed as their infection may be adversely having an impact on the body repeatedly. Also their enlarged size may be straining the breathing further in a child who is already suffering with asthma. So the children who suffer with asthma may be recommended a removal of tonsils if the asthma is of extrinsic type and is caused by allergens.
Sometimes in the children who suffer from intrinsic asthma too this may be recommended as the body becomes weakened and is susceptible to attacks from various organisms. But this is controversial because tonsils are the first line of defence and removing them may not be such a good thing to do, at least in theory. But in practice the tonsils may be a cause for many infections and it is best to get rid of the source of infections.
Some of the views regarding the question of removal of tonsils are:
1The tonsils act at the site and later as a source of infection in the body. Their removal would influence the course of asthma favourably and even prevent its occurrence in a susceptible individual.
2The tonsils prevent the spread of infection from the nose and throat into the bronchi and the lungs so their removal would create situations of mild asthma developing into severe one. In susceptible individuals symptoms of asthma may even precipitate due to tonsillectomy.
3The presence or absence of tonsils actually does not make any difference over the allergic state of an individual. So technically removal of tonsils cannot precipitate the symptoms of asthma according to this view.
With reference to the varied views mentioned above it seems that the most appropriate path for the surgery for the removal of tonsils would not only be an individual choice but also may mean that a certain individual needs to be studied for previous history of infections, process of treatment and pathway of recovering from the infections before any decisions can be taken.
If there is evidence of recurring infections of throat and the infections seem to be localised in the tonsils then this may be the only option provided there are no other serious complications. Children in the younger stages are more susceptible to repeated infections of throat and the chest so to understand that the infections are localised to the tonsil area is very important before taking the decision of tonsillectomy.
Another point to be thoroughly understood is that removal of infected tonsils may have no effect whatsoever over the condition of asthma of the child. The removal of tonsils may have effect on the associated conditions of aggravation of the symptoms because of the effect that the associated allergens may be having on the situation.
This is also true only to the extrinsic type of asthma, but it shall have none or very little effect on the intrinsic type of asthma. Some doctors feel that the benefits associated with tonsillectomy are most evident in the first year of surgery and later they dissipate and after some time there remains no difference between the asthmatic children who underwent surgery and the asthmatic children who did not undergo any surgery.

I wore Christmas socks yesterday because they were the only pair I could find. The festive, red and green twosome sat perfectly folded in my drawer amidst single distant relatives of tan and black. I have often spotted this merry sock marriage in times of need and had to reject them because the season was wrong. How wacky would that look - wearing Christmas socks in October, or February? Yesterday, however, the season was right.


Why so opposed to wearing them? The simple answer is they didn't match my top. The complicated answer is my top was black. And no, there is nothing near being in plain view in my closet that is the least bit Christmasy. There might not be anything at all. Now, now - Don't go thinking that this coupled with the fact that the Grinch who Stole Christmas is my new ringtone means I'm a Grinch! Except occasionally!


Nothing like Christmas socks sheds light better on how inadequate I am by societal standards at doing Christmas. I'm less a Grinch and more, a Charlie Brown in the weeks preceding Christmas. Some days I get it; some days I don't, and will focus too much with bitter irony on everything from how rude people are when trying to beat you to a parking space at the mall to songs lyrics that talk about snow as the vital ingredient to achieving holiday spirit. I'm forgetful. I'm late. And I feel bad about how many loved ones I've probably offended through the years.

I knew better than to hope that just because I'm on ADD medication that I would now become a superwomen who has all of her shopping done and owns a labeled box pulled down from the attic filled with Christmas attire. But I held out a glimmer of hope for a small Advent miracle: that perhaps the whole sock situation in our house would be a thing of Christmas Past. But apparently, it's not. One minute I'm almost ready to leave for the girl's piano recital; the next I'm considering rooting in my closet, berating myself for not having even one suitable Christmas sweater to match my socks.

For those of us who are still in the trenches, fighting for change and democracy across the entire African continent, this is our victory. One which for now we will savor and celebrate.

WARNINGS / PRECAUTIONS
Concerns related to adverse effects: Angina: Discontinue if symptoms of angina occur or worsen. Floppy iris syndrome: Intraoperative floppy iris syndrome has been observed in cataract surgery patients who were on or were previously treated with alpha1-blockers; causality has not been established and there appears to be no benefit in discontinuing alpha-blocker therapy prior to surgery. Orthostatic hypotension/syncope: May cause significant orthostatic hypotension and syncope, especially with first dose; anticipate a similar effect if therapy is interrupted for a few days, if dosage is rapidly increased, or if another antihypertensive drug (particularly vasodilators) or a PDE5 inhibitor is introduced. Patients should be cautioned about performing hazardous tasks when starting new therapy or adjusting dosage upward.

Disease-related concerns: Hepatic impairment: Use with caution in patients with mild hepatic impairment; not recommended in moderate to severe impairment. Prostate cancer: Should rule out prostatic carcinoma before beginning therapy. Renal impairment: Use with caution in patients with renal impairment.

Special populations: Pediatrics: Safety and efficacy have not been established in children.

Other warning/precautions: Antihypertensive agent: Not intended for use as an antihypertensive drug.

DRUG INTERACTIONS Substrate of CYP3A4 (major)

CYP3A4 inducers: CYP3A4 inducers may decrease the levels/effects of alfuzosin. Example inducers include aminoglutethimide, carbamazepine, nafcillin, nevirapine, phenobarbital, phenytoin, and rifamycins.

CYP3A4 inhibitors: May increase the levels/effects of alfuzosin. Example inhibitors include azole antifungals, clarithromycin, diclofenac, doxycycline, erythromycin, imatinib, isoniazid, nefazodone, nicardipine, propofol, protease inhibitors, quinidine, telithromycin, and verapamil. Concurrent use of itraconazole, ketoconazole, or ritonavir is contraindicated.

ETHANOL / NUTRITION / HERB INTERACTIONS Food: Food increases the extent of absorption.

MECHANISM OF ACTION An antagonist of alpha1-adrenoreceptors in the lower urinary tract. Smooth muscle tone is mediated by the sympathetic nervous stimulation of alpha1-adrenoreceptors, which are abundant in the prostate, prostatic capsule, prostatic urethra, and bladder neck. Blockade of these adrenoreceptors can cause smooth muscles in the bladder neck and prostate to relax, resulting in an improvement in urine flow rate and a reduction in symptoms of BPH.
