Most of the computers today use the idea of Stored Program Concept that was proposed by Dr. Jon Von Neumann in 1945 in his paper Theory and Techniques of Electronic Digital Computers. The concept is that the program (a set of instructions which perform a task) as well as its associated data should be stored in the main memory of the computer.
The CPU fetches the instructions comprising the program and the required data from the memory and performs the desired operations. Instructions are normally executed in a sequence in which they are stored, although it is possible to have different deviations as in the case of branching. Thus the functioning of a computer is completely under the control of the stored program. However, there are possibilities of external interruption by the user or by devices connected to the computer.
Hence, the computer performs the tasks according to the stored program with high speed and accuracy.
The Von Neumann computer architecture is based on the following three key concepts:
1. Data and programs are stored in the main memory (usually in the RAM).
2. The memory contents are addressable by locations.
3. Instructions are executed in sequence until explicitly instructed to do otherwise.
Von Neumann bottleneck
The separation between the CPU and memory leads to the Von Neumann bottleneck, the limited throughput (data transfer rate) between the CPU and memory compared to the amount of memory. In most modern computers, throughput is much smaller than the rate at which the CPU can work. This seriously limits the effective processing speed when the CPU is required to perform minimal processing on large amounts of data. The CPU is continuously forced to wait for needed data to be transferred to or from memory. Since CPU speed and memory size have increased much faster than the throughput between them, the bottleneck has become more of a problem.
The term Von Neumann bottleneck was coined by John Backus in his 1977 ACM Turing award lecture. According to Backus:
Surely there must be a less primitive way of making big changes in the store than by pushing vast numbers of words back and forth through the Von Neumann bottleneck. Not only is this tube a literal bottleneck for the data traffic of a problem, but, more importantly, it is an intellectual bottleneck that has kept us tied to word-at-a-time thinking instead of encouraging us to think in terms of the larger conceptual units of the task at hand. Thus programming is basically planning and detailing the enormous traffic of words through the Von Neumann bottleneck, and much of that traffic concerns not significant data itself, but where to find it.
The performance problem is reduced by a cache between the CPU and the main memory, and by the development of branch prediction algorithms. It is less clear whether the intellectual bottleneck that Backus criticized has changed much since 1977. Backuss proposed solution has not had a major influence. Modern functional programming and object-oriented programming are much less geared towards pushing vast numbers of words back and forth than earlier languages like Fortran were, but internally, that is still what computers spend much of their time doing.

Escitalopram (Lexapro by Forest Laboratories in the United States and elsewhere by Lundbeck as Cipralex, Sipralexa, Seroplex and also as Lexapro) is an antidepressant of the selective serotonin reuptake inhibitor (SSRI) class. It is approved for the treatment of major depressive disorder and generalized anxiety disorder; other indications include social anxiety disorder, panic disorder and obsessive-compulsive disorder. Of the SSRIs currently on the market escitalopram has the highest affinity for the human serotonin transporter (SERT). Another enantiomer of citalopram (R-citalopram) counteracts to a certain degree the serotonin-enhancing action of escitalopram. As a result, escitalopram is a more potent antidepressant than citalopram, which is a mixture of escitalopram and R-citalopram. In order to explain this phenomenon, researchers from Lundbeck proposed that escitalopram enhances its own binding via an additional interaction with another allosteric site on the transporter. Further research by the same group showed that R-citalopram also enhances binding of escitalopram, and therefore the allosteric interaction cannot explain the observed counteracting effect. In the most recent paper, however, the same authors again reversed their findings and reported that R-citalopram decreases binding of escitalopram to the transporter. Although allosteric binding of escitalopram to the serotonin transporter is of unquestionable research interest, its clinical relevance is unclear since the binding of escitalopram to the allosteric site is at least 1000 times weaker than to the primary binding site.
In vitro studies using human liver microsomes indicated that CYP3A4 and CYP2C19 are the primary isozymes involved in the N-demethylation of escitalopram.

Agoraphobia is a condition where the sufferer becomes anxious in environments that are unfamiliar or where he or she perceives that they have little control. Triggers for this anxiety may include crowds, wide open spaces, or traveling (even short distances). This anxiety is often compounded by a fear of social embarrassment, as the agoraphobic fears the onset of a panic attack and appearing distraught in public.
Agoraphobics may experience panic attacks in situations where they feel trapped, insecure, out of control or too far from their personal comfort zone. In severe cases, an agoraphobic may be confined to his or her home. Many people with agoraphobia are comfortable seeing visitors in a defined space they feel they can control. Such people may live for years without leaving their homes, while happily seeing visitors in and working from their personal safety zones. If the agoraphobic leaves his or her safety zone, they may experience a panic attack.
